Objective Chronic ankle instability (CAI) is associated with biomechanical deficits, reduced dynamic postural control, and impaired functional movement. This study examined the effects of an 8-week exercise-based rehabilitation program on biomechanical and functional performance in athletes with CAI.
Methods Twenty-four male athletes with CAI were randomly allocated to an experimental group (EG; n = 12) or a control group (CG; n = 12). The EG completed a progressive, structured rehabilitation program involving mobility, proprioceptive training, muscle strengthening, and neuromuscular control exercises three times per week for 8 weeks. The CG continued their usual sport-specific training and daily physical activities. Outcome measures included peak ankle dorsiflexion during drop-jump landing, isokinetic evertor peak torque at 60°/s, Y-Balance composite score, and Functional Movement Screen (FMS) total score. Data were analyzed using one-way analysis of covariance (ANCOVA), with the corresponding pre-test score entered as a covariate for each outcome.
Results ANCOVA revealed significant between-group differences in all four outcomes after adjustment for pre-test scores (all p < 0.001), with large effect sizes (partial η² = 0.505–0.564). Adjusted post-test means were higher in the EG than in the CG for peak ankle dorsiflexion (24.62° vs. 19.08°), evertor peak torque (23.55 vs. 18.05 N·m), Y-Balance composite score (92.32% vs. 84.98%), and FMS total score (16.28 vs. 14.02 points).
Conclusion An 8-week progressive, multidimensional exercise-based rehabilitation program was associated with higher adjusted post-test values across the four measured biomechanical and functional outcomes in male athletes with CAI. These findings indicate short-term improvements in peak ankle dorsiflexion, evertor peak torque, Y-Balance composite score, and FMS total score following the rehabilitation program.
